The engine is not reaching the temperature the thermostat is supposed to open at. A stuck-open thermostat is the usual part. A low coolant level or a lying temp sensor is the other story.
Do not throw a water pump from P0128. Check the first-running temp and the coolant level.
DIY
On many fours this is a driveway job if you can catch the drained coolant. Housing-integrated stats are messier.
